CONGRATULATIONS TO THE 2010 SANTA CRUZ LITTLE LEAGUE ALL-STARS!!! Very Important Information Regarding All-Stars: Thursday, June 17th is the MANDATORY REGISTRATION for 2010 All-Stars. Even though everyone registered for the regular season, All-Stars MUST REGISTER AGAIN. This is a rule of Little League International and we, as members, must abide by it. All-Star registration will be Thursday, June 17th, from 7:30pm to 8:30pm at Harvey West Park Club House (in deep, deep left field at Field 1). You will register with your Manager of your All Star team. Each player and parent MUST BE PREPARED and BRING THE FOLLOWING: 1. Updated registration form signed and dated for June 15th or later (form can be found on-line)
2. Copy of birth certificate
3. Updated Medical Release Forms signed and dated for June 15th or later (form can be found on-line)
Each player MUST also supply THREE of the following proofs of residency for their family:
1. Driver's License
2. Voter's Registration
3. School records (current)
4. Utility bills (gas, electric, phone, mobile, water/sewer, etc..,)
5. Medical records
6. Internet, cable or satellite records
7. Insurance records
Blank forms will be available at Harvey West for the player registration and the medical release forms if you do not have access to a printer.
